Registered Dietician - Corporate

The Plaza Team is excited to announce that due to the growth of the organization, we are looking for a Registered Dietician to join our team!

This position will be working out of the Corporate location in downtown Honolulu.

The Registered Dietician will be responsible for working with The Plaza communities in providing diet-related improvement methods for their Residents. They will be required to collaborate and provide nutrition options for the Food Service Department on a frequent basis.

The following are essential functions of the job:

  • Work with the Executive Chef in ensuring that The Plaza's menu options meet all required State of Hawaii, Department of Health nutritional requirements for seniors.
  • Support The Plaza communities with nutritional assessments, evaluations, and counseling based on the Residents' health metrics and habits.

  • Assists the Nursing Department to analyzing Residents' health status and identifying nutritional plans to assist in goals and improvement.
  • Assist in creating meal plans that take into account Residents' life circumstances and chronic medical conditions.
  • Follow up on Residents' progression through meal plans and ensure they are modified in accordance with all quality standards of care.
  • Provide dietary consultations to Residents and Families when their medical conditions are identified as requiring an intervention.
  • Provides educational sessions to Plaza Team Members, Residents, and Families as necessary on diet specific topics (i.e. how to maintain a balanced diet).
  • Works towards providing quality improvements to the food service menu to better improve Residents well-being.
  • Must be able to communicate well with others, Managers/Directors, Team Members, Residents/Families.
  • Comply with all HIPAA requirements to protect the confidentiality of residents' PHI (Protected Health Information).
  • Will be required to possess excellent oral and written communication skills and have moderate computer skills.
  • Performs other duties and responsibilities as assigned.

Required

  • Must have a Bachelors Degree in Nutrition or related field.
  • Must have a Registered Dietician Certification in the State of Hawaii.
  • Current Tuberculosis Clearance, according to the State of Hawaii Department of Health.
  • CPR and First Aid Certified.

Preferred

  • Certification or specializes in senior specific medical conditions (i.e. Diabetes).

Physical Requirements

Dietician must be able to travel from Plaza community to community to meet with Plaza Team, Residents & Families.

Frequent standing and walking, bending, pushing, pulling, and occasional lifting. Frequent sitting and working on computer/laptop and consulting with others. In addition, must be free of communicable disease and meet company policies and procedures regarding necessary physical health.

Float Director of Nursing - Full Time (exempt)

The Plaza Assisted Living is looking for a new nurse management role, Float Director of Nursing (FDON).

We are very excited to open this management role that will work within our Plaza communities and will be responsible for supporting the nursing department at the assigned Plaza community.

Additionally, in the absence of nursing leadership, the FDON works as an extension of the Director of Quality Assurance to maintain compliance and management of the nursing department. The main responsibilities include, but are not limited to, supporting the assigned community Director of Nursing (DON) and/or Administrator to maintain compliance with state and federal laws and community policies and procedures, specifically ensuring quality of care is being provided to the residents.

The FDON will be required to cover and travel between their assigned communities and will communicate directly to the community Administrator/DON under the supervision of The Director of Quality Assurance.

The following are essential functions of the job:

  • In the absence of a DON, required to be on-call twenty-four (24) hours a day for assigned community or communities. If a DON is present, required to be on-call twenty-four (24) hours a say for the DON or Administrator.
  • Performs assessments for each resident, prior to admission/readmission to determine if the prospective resident's care needs can be appropriately met by the community and every 6 months or as needed for acute change in condition.
  • Ensure that Resident Service Plans are created and maintained according to assessments and develops Care Communication for resident, family members and staff.
  • Manages the out-of-facility residents, including any hospital or rehabilitation stay, to ensure readmission assessment, orders, and return is completed timely.
  • Ensures proper documentation of care delivery as ordered by the physician in accordance with federal requirements, state regulations and The Plaza policies and procedures; recognizing any deviation from Resident Service Plans, Department of Health standards and Plaza policies and procedures. This also includes monitoring the electronic medical records system for compliance and service delivery (i.e. POC).
  • Assists in monitoring residents' nutritional status to ensure care needs are met in accordance with Resident Service Plans, Department of Health standards and The Plaza policies and procedures. This includes quality checks for monthly weights, Registered Dietician consultations, and any condition that would require an intervention
  • Performing rounds frequently throughout the day to observe efficiency of care service delivery; interviews staff, residents, families or other parties of interest as appropriate.
  • Oversees and supports Medication Management Program that includes, assessing individual residents to determine ability for self-administration of medication, provides oversight of medication storage, and administration as appropriate.
  • Routinely monitors and audits medication management program and ensures medications/treatments are administered and documented by nursing and care staff as ordered by the physician in accordance with federal requirements, state regulations and Plaza policies and procedures. This also includes managing any Pharmacy Review or recommendations.
  • Manages the health promotion and disease prevention programs that includes administration of immunization as appropriate, development of protocols for infection disease management (e.g. flu outbreaks, tuberculosis etc.), and ensuring proper isolation procedures are being met.
  • Monitors and reviews the community's Incident Reports and assists in resolving and prevention.
  • Assists and manages, when necessary, the nursing department. This includes hiring, on-boarding, scheduling, payroll processing, trainings, team member relations/issues, and disciplinary and performance evaluations.
  • Assists in setting up proper and necessary nursing department trainings. This can include identifying accident prevention procedures to protect team members from illness and injury, completion of Medication Aide Skills Training Assessment (initially and annually), on-the-job trainings, and any other training that is deemed necessary for the nursing department.
  • Maintain supplies and staffing labor budget in accordance with their current community census.
  • Manages and participates in developing and maintaining a Quality Assurance Program in coordination with Director of Quality Assurance, Administrator, and other departmental staff.
  • As necessary will assist in promoting customer service and hospitality and responses to and adequately resolve complaints or concerns from residents or families about nursing services.
  • Is present and available when internal, third party and state inspectors visit the community.
  • Is familiar with disaster and fire plans as specified in policies and procedures manual and assists with community emergencies as instructed.
  • Adheres to all employment requirements established in the Team Member Handbook, the Operating Policies and the Procedural Manuals.
  • Performs other duties as assigned by the community Administrator or direct supervisor.

Supervisory - as necessary:

  • Participates in the recruitment, interviewing and selection of nursing and care staff personnel.
  • Schedules staffing needs within budget as business needs demands minimizing the use of third-party agencies where possible.
  • Counsels team members when required in a fair and consistent manner while adhering to The Plaza policies and procedures and state/federal employment laws.
  • Supervises nursing/care staff and regularly evaluates their performance, frequently engages team members (e.g. Initial/30/90/biannual/annual).
  • Conducts regular shift reports, daily meetings and other modes of communication to disseminate information, interpret policies and procedures or address concerns as needed.
  • Assures resident nursing department and work areas are clean, neat and orderly. Develops and provides ongoing in-service training for all care staff as relevant to the needs of the resident and the community.

Required

  • Current State of Hawaii Nursing License RN or higher (in good standing).
  • Current Tuberculosis Clearance, according to the State of Hawaii Department of Health regulations.
  • CPR and First Aid Certification.
  • One (1) years' experience in Director of Nursing role within The Plaza communities or equivalent.

Preferred

  • Minimum of three (3) years' experience in an assisted living facility or long-term care.Starting salary is $90,000 per year; negotiable based on prior experience.

Staff Accountant - Corporate Office

Job Purpose: The Staff Accountant is responsible for supporting all accounting functions, to include but not limited to; Accounts Payable (AP), Accounts Receivable (AR), Payroll, General Ledger, Bank and Credit Card account reconciliation, completing periodic book closing and preparation of financial reports, and assisting with annual tax and budget preparations.

The following are essential functions of the job:

  • Maintains and processes timely and accurate records for all payables and receivables, in accordance with The Plaza's Accounting Policies and Procedures.
  • Reviews, processes, and maintains weekly AP invoices and payments via check, ePayables, and/or credit card.
  • Reconciles vendor account statements to ensure timely payments.
  • Maintains AR by updating cash receipts, transferring funds from trust bank account to operating account, updating/invoicing resident accounts, reconciling and processing monthly billing transactions.
  • Reviews and posts payroll; reconciles quarterly benefit reports.
  • Prepares, records, and posts manual employee payroll checks as requested.
  • Assists with internal management of The Plaza's Credit Card Program, includes expense reporting and data imports into the accounting software system.
  • Prepares and reports monthly General Excise Tax (GET) and process tax payments.
  • Reviews and posts monthly recurring journal entries and books prepaid expense journal entries.
  • Reviews and reconciles bank and credit card accounts.
  • Review and reconciles general ledger accounts and census information periodically for the preparation of financial statements.
  • Prepares and analyzes monthly financials packets for accuracy and completeness.
  • Communicates and provides accounting support to community Business Office Managers and Administrators to ensure accurate billing and payments of resident and vendor transactions.
  • Works collaboratively with other departments, attends community meetings, trainings and in-services as assigned.
  • Adheres to all employment requirements established in the Team Member Handbook, the Operating Policies and the Procedural Manuals.
  • Performs other duties as assigned by Controller


Required:

  • Bachelor's Degree from an accredited 4‐year university and/or relevant work experience equivalency.
  • Proficiency with business, accounting and office software, and office equipment.

Preferred:

  • Bachelor's Degree in Accounting.
  • Three (3) years of experience in Staff Accountant or similar industry position.
  • One (1) year experience processing high volume APs, ARs, and billing transactions.
  • Familiarity with PointClickCare, SAGE Intacct, and Enquire.

  • Notifies the Director of Nursing and Administrator, physician and family when significant changes in condition occur and takes appropriate action as directed.
  • Assists in emergency situations involving the residents.
  • Responsible for Incident Report writing, in a timely manner and followed-up with an appropriate investigation.
  • Ensures medications are ordered and received from pharmacy in a timely manner and handled in accordance with company policy.
  • Ensures appropriate contact with physicians occurs regarding written and verbal orders (this includes properly transcribing physicians' orders with accuracy and assuring follow-up as indicated) in accordance with regulations, standard nursing practice and established community policies and procedures.
  • Functions as a floor nurse, as needed, including the passing of any PRN medication.
  • Makes daily rounds to determine unit effectiveness and identifies needs and concerns.
  • Identifies and communicates trends on the care floor, assists the Director of Nursing and Administrator with a plan of action to follow through.
  • Makes recommendations regarding policies and procedures to maintain the care program in compliance with state and federal laws and regulations, while offering the best possible care to residents.
  • Responsible for ensuring care department sustains compliance with state and federal laws and community policies and procedures.
  • Completes forms and reports as requested by Director of Nursing or Administrator.
  • Collaborates efficiently with state inspectors regarding surveys and care complaints.
  • Assures residents service department, supplies, equipment and work areas are clean, neat and orderly and submits requests for supplies within departmental budgets.
  • Maintains good rapport with residents, families, visitors, and community members and resolves resident, family and community concerns promptly.
  • Performs other duties as directed by the Director of Nursing or/and the Administrator.

Supervisory

  • Oversees daily nurse delegation program for all floors responsible for.
  • Schedules nursing and resident care staff to ensure appropriate resident/staff ratio.
  • Enforces the attendance policies if needed, and replaces team members as needed.
